Excellent Brands Requirements 

1.      Must be a member of the Oriental Food Association;

2.  Nominated by 2 OFA members;

3.      Possess one of the following industry quality assurance standards:

a.      FDA

b.      HACCP

c.      ISO

d.      GMP

4.      Product liability insurance of $1 million or more, naming OFA as an additional insured;

5.      Product expiration date, nutritional content and ingredients information should be clear, accurate, and easy to find and understand;

6.  Comply with product recall procedure and all regulation established by rellevant governmental agencies.

I hereby sign and certify that we abide with the above requirements. In the event we fail to comply with the requirements, we understand that Oriental Food Association has the right to remove us from the “OFA Excellent Brands”.
